Efficacy of Combined Septal Extension and Derotation Grafts in Asian Rhinoplasty: A Quantitative Analysis of Tip Projection and Stability High Impact

A quantitative comparison of three nasal tip-plasty techniques—derotation graft with columellar strut (DG+CS), septal extension graft alone (SEG only),
and combined septal extension with derotation graft (SEG+DG)—for short nose correction in Asian rhinoplasty. In 324 single-surgeon patients (2015–2022),
standardized lateral photographs were used to measure nasal length, tip projection, and nasolabial angle (normalized to facial length). SEG+DG showed the
most consistent and durable long-term improvement with the lowest variability (P<.001) and the lowest revision rate (6.5% vs 10.6% for SEG only and
14.3% for DG+CS), offering the most reliable structural support for short nose deformity.

A Novel Method for Full-thickness Skin Graft Fixation Over the Fingers in Complex Hand Reconstruction

This study introduces a simple and effective technique to secure full-thickness skin grafts (FTSG) over the fingers using a self-adherent
wrap elastic bandage (3M Coban), eliminating the need for traditional tie-over dressing in complex hand reconstruction. Fixation of a single
finger takes only 1–1.5 minutes (versus about 1 hour for conventional tie-over of the whole hand). Fifty-three patients with a total of 74
hands with burn deformities were treated; all finger grafts took well (estimated take rate >95%) without obvious hematoma, necrosis, or
infection, over a mean follow-up of 27.82 months.

The research analyzes differences between the two surgical methods in terms of nasal function recovery and appearance improvement,
providing evidence-based guidance for clinicians in selecting appropriate surgical strategies. Results demonstrate that combined surgery
achieves superior overall outcomes in complex crooked nose deformity treatment.
A Novel T-Shaped Design of the Forehead Flap in Columella Reconstruction High Impact

This study developed an innovative T-shaped forehead flap design for columella reconstruction. Addressing the issue of flap dehiscence
between the forehead flap and upper lip, the authors proposed a T-shaped modification. By creating a subcutaneous pocket and inserting
the de-epithelialized distal part of the flap, greater contact and connection between tissues is achieved. Treatment results from 23 patients
show that all wounds healed smoothly within 2 weeks, with no flap edge dehiscence or other complications.

Risk factors for postoperative adverse airway events in patients with primary oral cancer undergoing reconstruction without prophylactic tracheostomy High Impact

This retrospective study analyzed risk factors for postoperative adverse airway events in 379 primary oral cancer patients who underwent
tumor resection and free flap reconstruction without prophylactic tracheostomy. The study found that 7.6% of patients experienced
postoperative adverse airway events, with patient age, tumor location, cross-midline segmental mandibulectomy, and lower PF ratio
as independent risk factors. This research provides important evidence for identifying high-risk patients and developing individualized
airway management strategies.

Alar Crease Creation Suture in Nasal Reconstruction High Impact

Developed an innovative suturing technique for creating alar creases in nasal reconstruction. The study applied this technique
to 22 consecutive patients undergoing nasal reconstruction with forehead flaps, with an average follow-up of 3 years and 7 months.
Results demonstrate that this modified interrupted inverted horizontal mattress suturing technique can safely, directly,
and reliably create aesthetic alar grooves.

Machine learning approach for predicting inhalation injury in patients with burns High Impact

Developed the first machine learning tool for differentiating between mild and severe inhalation injury,
and the absence/presence of inhalation injury in patients with burns. The model combines clinical parameters
and imaging features with excellent discrimination (AUC: 0.883 and 0.862), helping clinicians when bronchoscopy
is not immediately available.
Simultaneous free flap and forehead flap for nasal reconstruction High Impact

This study demonstrates the combination of preliminary free tissue transfer for nasal lining restoration
with paramedian forehead flap for nasal skin replacement in the same stage. Results show this approach
can shorten the long process and achieve satisfactory reconstruction in complex nasal reconstruction.
Average operation time was 9.5 hours with 29.6 months follow-up.

A Comparative Study Between Classic Derotation Graft and Novel Double V Cutting Folded Derotation Graft High Impact

This study introduces an innovative nasal tip lengthening technique - the "Double V Cutting Folded Derotation Graft" (DVCFD graft),
which is a method for nasal tip lengthening in aesthetic rhinoplasty with strong holding force and efficient cartilage use.
A retrospective study of 101 patients compared traditional derotation grafts (49 cases) and DVCFD grafts (52 cases) in cosmetic tip plasty.
The study measured tip projection, columella-labial angle, and nasolabial angle changes through clinical photography, demonstrating
the superiority of DVCFD grafts.
The Upper Buccal Musculomucosal Flap for Nasal Lining and Columellar Defect Reconstruction

This study reports experience using the upper buccal musculomucosal (UBMM) flap for reconstruction of full-thickness columellar
or nasal lining defects. Ten patients underwent UBMM flap reconstruction of columellar or nasal lining defects, with or without
composite grafting, between December 2014 and the study period. Due to limited availability of healthy local tissue for nasal
lining reconstruction after multiple surgeries, the UBMM flap serves as an applicable choice to restore columellar or nasal
lining defects, with minimal donor site morbidity and no visible external scarring.
